HIV Reservoirs: Four main questions for 2011

Toulon, France, January 5, 2011 -- Despite effective antiretroviral combinations (HAART) capable of totally blocking the Human Immunodeficiency Virus (HIV) replication, this retrovirus persists life-long in patients due to the existence of hidden reservoirs. Major advances have been made since the first description of these HIV reservoirs in 1997 but a handful of fundamental questions remains to be elucidated before trying to find a cure.

Question 1: What is ‘the’ HIV Reservoir and Where is it Located?
The majority of the long-term HIV reservoir is situated in CD4+ T lymphocytes. These cells keep the memory of the immune system and are long-lived. We also know that within these CD4+ lymphocytes, the HIV reservoir is located in the central memory and transitional memory sub-populations. The HIV reservoir is 10 fold higher in the gut than in the blood and, in the brain, infected astrocytes could be the principal cells responsible for HIV persistence.
On the contrary, data on other HIV reservoir cells are scarce (Stem cells? Macrophages? Other sorts of cells?) as well as on different body compartments than the gut and the brain.

Question 2: What other factors than the virus play a role in the HIV Reservoir?
There are at least some recognized genetic factors and immune factors. For example, ‘elite controllers’ (patients controlling HIV replication themselves without the need of HAART) have a better CD8 antiviral response, leading to a kind of protection of the central memory CD4+ T cells, and this is associated with the presence of HLA B27 and B57 markers.
However, the exact nature of the immune response that could be able to control a very tiny HIV reservoir remains uncertain.

Question 3: What current HAART can do on the HIV reservoir?
The HIV reservoir is established very early, at the time of acute HIV infection. Treating HIV with HAART at the chronic stage of the disease is unable to decrease the HIV reservoir. However, it is highly likely that initiating HAART very early, at acute infection, limits the HIV reservoir size and the damages done to the immune system. Nevertheless, the clinical benefits of this early approach still need to be proven. It is also unknown if there is a threshold below which we can drive the HIV reservoir size and this one will not rekindle when HAART is discontinued. Finally, almost nothing is known on antiretroviral drugs activity in cells from different tissues.

Question 4: Can  the HIV Reservoir be targeted and how?
The most advanced theory is the HIV reservoir ‘purge’, i.e. activating latently infected cells in the presence of HAART to make them die or killed by the immune system. The main candidates are Histone Deacetylase Inhibitors (HDACi) and Interleukin-7 (IL-7). There are concerns that IL-7 would not be strong enough to reactivate the HIV reservoir, or even help expand it, and that HDACi could put the brain HIV reservoir beyond control. Preliminary results of small pilot studies testing these new strategies are urgently needed.

This way the competition is lesser and the chances of securing a job are much higher as compared to group interviewing.

Personal evaluation will help in salary negotiation. It needs a bit of skill to ensure that both parties are comfortable with whatever is set. Similarly important are questions on insurance and liabilities in the negotiation process. It is paramount to clarify issues that may come up in the future.

Depending on the job, there may be different things that are required of the interviewee and knowing them prior to the interview process might help.

In developing TravelTools, Mattsonmaps firmly believes that the industry needs a software application for common tools as much as it needs CMS applications for site content. Mattsonmaps plans on expanding the tool market horizontally in that its software represents economies that can be passed along to even small DMOs and organizations of all sizes that are feeling the crunch of a slowing global economy.

Given the importance and expense associated with managing content on typical DMO websites, the competitive environment for servicing the first of these needs—content management systems—is exceedingly fierce bringing widespread economies based on standardization, ease of use, and the streamlining of routine tasks.

Curiously, comprehensive solutions for web tools don't exist. As a default, organizations employ individual contractors or ask their own workers to code tool widgets on an installation-by-installation, solution-by-solution basis. To be fair, many of these solutions work like crazy, but issues of tool development, management and evolution have not benefited from essential "systems thinking approaches" in the same way that the industry has benefited from solutions surrounding web content and member management. Company President Mark Mattson wonders why. "If tools are essential to promoting a multi-billion dollar industry, it would make sense to have applications that are up to the task.

The company’s early success draws upon joint marketing research with the Rohrer Business School at Rowan University during which senior marketing students surveyed 650 destination marketing organizations and found that more than half wanted to upgrade their sites. Most of those surveyed wanted tools, but couldn't afford them or didn't know where to find solutions".

The survey also found that tool solutions are not easily understandable on a technological level and that they are expensive to develop, upgrade, and operate. 87% of the survey’s respondents paid outsiders to develop tool applications. Furthermore, many felt trapped by their purchases—feeling locked into "existing solutions" as their financial resources diminished and their needs for tools increased. Moreover, the existing approach to tool acquisition and development delivered varying levels of satisfaction in terms of product equity, agility, or standardized efficiencies. At the end of the day, 64% of respondents complained of a lack of manpower despite costly investments in tool technologies that were originally made for the very purpose of decreasing organizational workloads. 32% were also displeased with the technology that they had purchased and wanted "newer solutions" and 30% complained about pricing and customer service associated with their tool purchases or acquisitions.

Mattson adds, "Since tools can't be bought as unified software packages the way that "content management systems" can, the costs and inefficiencies associated with custom software development are far-reaching in terms of an organization’s ability to compete in a global climate of economic scarcity. Our gut feelings are supported by our survey responses."

Consider: 69% of respondents said that they knew what they needed, but lacked the resources to orchestrate better technology solutions in an online market that has exploded to 80% of the 120 million U.S. travelers in 2009. As a result, 60% of respondent websites lacked "truly interactive" mapping capacities, 53% lacked itinerary builders, and 70% were incapable of gathering important marketing data from visitors.

To bridge the gap between what destination marketers need and the limits imposed by "scarce resources" Mattson concluded that the destination marketing industry needed an integrated software solution for destination marketing tools. Furthermore, he has developed one called TravelTools— a proprietary web-based software application that monetizes, streamlines, and economizes the day-to-day operations of websites run by destination marketing organizations. Within 72 hours, Mattson’s team will install an easy-to-use, totally integrated software solution that manages a DMO site’s membership listings, attractions, events, visitor itineraries, maps, newsletters, sponsorships, advertisements, announcements, hot deals, marketing data, and conferences. Not only that, the system is totally integrated with client data and graphics. Finally, because the system is so economical, Mattson hopes to reach out to the hundreds of smaller DMOs that can't afford the expensive systems that their larger competitors can. "We want to democratize destination marketing by bringing world-class capacity to everyone. Smaller and larger organizations alike place the same value on tourism dollars. It’s not right that some destinations show well in the market while others are priced-out missing opportunities to promote great, smaller places.”

IAPAM Announces 2011 Dates for its Aesthetic Medicine Symposiums with Botox Training

-- In the coming year, physicians focused on growth will continue to seek out the best botox training, and hCG for weight loss programs, particularly programs offering physician-lead, hands-on training in botox and the other minimally-invasive treatments. Biotage introduces a new sample preparation application note for the extraction of ethyl glucuronide from urine by LC-MS

Biotage Announces an Easy, Cost-Effective Approach to the Analysis of Ethyl Glucuronide (EtG) in Urine for Clinical and Forensic Toxicology Laboratories.

Uppsala, Sweden, December 17, 2010 -- Biotage® (STO: BIOT), a leading supplier of tools and technology for analytical and medicinal chemistry, introduced a new sample preparation application note for the extraction of ethyl glucuronide from urine by LC-MS.

EtG can be detected at very low levels and being a metabolite it is useful in distinguishing between ingested ethanol and that which is absorbed through the skin from ethanol containing products such as alcohol based hand washes. Rapid and reliable methods with robust extraction protocols are essential for analysis and quantitation of EtG from various matrices.

Advances in the manufacture of polymeric stationary phases and the recent release of EVOLUTE® AX, a mixed-mode ion exchange product, have allowed for the development of a more robust, cost-effective procedure. The protocol was developed on 96-well plates for high-throughput with volumes outlined for scale-up to cartridges in manual or automated systems.

EVOLUTE AX sample preparation products are available in a range of standard configurations and 2 different particle sizes (30 µm and 50 µm) to suit a wide variety of applications. The mixed-mode sorbent chemistry and optimized pore size minimizes retention of high molecular weight matrix components present in the biological fluid samples. Interferences including proteins, salts, non-ionizable molecules and phospholipids are removed during the interference elution step, thereby ensuring extremely clean extracts.
