HIV Reservoirs Cure Research – More Investments Are Needed
Toulon, France, January 12, 2010 -- With great effort and research into how to fight the HIV virus, important advancements in treatment have been made. What used to be a near-automatic death sentence is no longer the case. With the right mixture of medicines, an HIV-positive person can now live much longer than before. The HIV asymptomatic periods are now much longer than before as well, thanks to HIV research. While all of this great work does not mean an outright cure has been found, the fact that HIV-positive people can continue to live normal lives means extraordinary progress has been achieved.
Antiretroviral therapy has been very successful in lengthening the diseases free periods in those who are HIV-positive; however, HIV infection persists for life, and every time therapy is stopped, the HIV rekindles and becomes active again. These HIV reservoirs of viruses must be dealt with. If left unchecked for too long, the mechanisms leading to full blown AIDS start again and severe health problems occur. Therefore, more HIV research is needed to attack these reservoirs with new medications, and that can only happen if more research and development can be put into the project. HIV suppression has worked so far, but additional research could yield a medication that targets directly the HIV latency period. We must be committed to funding additional research.
Without a preventive vaccine on the horizon, HIV’s pandemic is not controlled and could turn into something far more major and devastating. This isn’t just a problem in a few industrialized nations; HIV is a worldwide problem, and it’ll take a global effort to eradicate this terrible disease. Scientists can do their part by continuing to do research on new strategies against HIV; doctors can evaluate the effectiveness of each medication patients can continue to use these products and explain to the doctors what works and what doesn’t. Also, government agencies can make it easier for scientists by showing a real and concrete political will to cure the disease.
In addition to political and medical support, HIV research requires a fair amount of financial support. Developing new kinds of drugs takes some time and huge financial efforts, but with a global commitment, the HIV reservoirs can be dealt with in timely manner, the HIV latency period can be therapeutically targeted, which means reaching a ‘functional cure’ at least for millions of people living with HIV on a regular basis. While HIV persistence is inevitable in those who are HIV-positive, the right treatment will make it seem as if the virus is not affecting them at all, and one of the goals is to allow these HIV-positive people to lead as normal a life as they possibly can. With more time, effort, and financial support, we can also achieve a cure and total HIV eradication. However, that journey begins with a single step in the right direction.

International Efforts for a Society Free of AIDS: HIV Reservoirs Control
Toulon, IL, January 07, 2010 -- HIV and Aids have been around for 3 decades and yet, few diseases have ever had such a profound effect: it has taken the lives of millions of people and hit virtually every nation in the world. In addition, there is still no cure and Doctors and researcher are still fighting to create a society without AIDS.
All over the globe teams of scientists are actually dedicating their time and resources to finding a way to tackle HIV reservoirs as the virus survives hidden in these sanctuary sites despite therapy. What is it about this particular disease that makes it so difficult to cure?
HIV is impossible to eradicate because it establishes reservoirs. Although the disease can be slowed with antiretroviral therapy, HIV cannot be totally eradicated from the body. One of the main reasons is that HIV hides from drugs and immune surveillance in different organs and regions of the body, such as the brain and the gut. Doctors are not even yet able to determine and identify all the places where the disease can hide. HIV is also established in a dormant state in some reservoir cells where it can last for decades.
The ability of HIV to hide and remain silent makes some people wonder if the disease will ever truly be "cured" one day. Even if a sterilizing cure, i.e. getting rid of the virus in an infected individual, seems currently out of reach, other strategies of ‘control’ are on the horizon. First, by treating HIV infection very early, at the time of acute infection, up to 15% of patients can sustain HIV replication control after therapy is subsequently stopped. HIV is still present in these patients but remains silent, causing no harm, despite discontinuation of antiretroviral drugs. Therefore, they have been transformed in “Elite Controllers”, these rare cases of HIV infection (less than 1% of infected individuals) where the body itself is able to control the virus without medication. Then, some researchers are testing strategies to “purge” the reservoir. They give antiretrovirals to the patient to block HIV replication, but also other molecules able to stimulate HIV replication…in the cells where the virus is dormant. They hope to bring these cells to be destroyed or to die due to HIV expression, whereas healthy cells are protected by antiretrovirals.
To hasten progress in HIV reservoirs cure or control, a coordination strategy is urgently needed. In that way, we launched in 2003 an international HIV reservoirs workshop, dedicated to researchers, which also is a think tank on this topic. New scientific results relevant to tackling HIV persistence were presented at the second workshop in 2005, more clinical results were shown at the following editions in 2007 and 2009.
Whether eradicating the virus (HIV sterilizing cure) or achieving long-term remission in the absence of continued therapy (HIV functional cure) will be possible one day remains currently unknown. The categories of the workshop sessions span from animal models, identifying the locations of HIV reservoirs in the body, the mechanisms of viral persistence, to potential therapeutic interventions and methods to evaluate their impact.
In 2007 and 2009 we were joined in our fight against HIV reservoirs by institutional support, both from the NIH (American National Institute of Health) and the ANRS (French Agency for AIDS Research). The workshop was also accredited by the Massachusetts School of Medicine. The 2011 HIV reservoirs workshop will be held in December in Saint Martin.

HIV Reservoirs: Four main questions for 2011
Toulon, France, January 5, 2011 -- Despite effective antiretroviral combinations (HAART) capable of totally blocking the Human Immunodeficiency Virus (HIV) replication, this retrovirus persists life-long in patients due to the existence of hidden reservoirs. Major advances have been made since the first description of these HIV reservoirs in 1997 but a handful of fundamental questions remains to be elucidated before trying to find a cure.
Question 1: What is ‘the’ HIV Reservoir and Where is it Located?
The majority of the long-term HIV reservoir is situated in CD4+ T lymphocytes. These cells keep the memory of the immune system and are long-lived. We also know that within these CD4+ lymphocytes, the HIV reservoir is located in the central memory and transitional memory sub-populations. The HIV reservoir is 10 fold higher in the gut than in the blood and, in the brain, infected astrocytes could be the principal cells responsible for HIV persistence.
On the contrary, data on other HIV reservoir cells are scarce (Stem cells? Macrophages? Other sorts of cells?) as well as on different body compartments than the gut and the brain.
Question 2: What other factors than the virus play a role in the HIV Reservoir?
There are at least some recognized genetic factors and immune factors. For example, ‘elite controllers’ (patients controlling HIV replication themselves without the need of HAART) have a better CD8 antiviral response, leading to a kind of protection of the central memory CD4+ T cells, and this is associated with the presence of HLA B27 and B57 markers.
However, the exact nature of the immune response that could be able to control a very tiny HIV reservoir remains uncertain.
Question 3: What current HAART can do on the HIV reservoir?
The HIV reservoir is established very early, at the time of acute HIV infection. Treating HIV with HAART at the chronic stage of the disease is unable to decrease the HIV reservoir. However, it is highly likely that initiating HAART very early, at acute infection, limits the HIV reservoir size and the damages done to the immune system. Nevertheless, the clinical benefits of this early approach still need to be proven. It is also unknown if there is a threshold below which we can drive the HIV reservoir size and this one will not rekindle when HAART is discontinued. Finally, almost nothing is known on antiretroviral drugs activity in cells from different tissues.
Question 4: Can the HIV Reservoir be targeted and how?
The most advanced theory is the HIV reservoir ‘purge’, i.e. activating latently infected cells in the presence of HAART to make them die or killed by the immune system. The main candidates are Histone Deacetylase Inhibitors (HDACi) and Interleukin-7 (IL-7). There are concerns that IL-7 would not be strong enough to reactivate the HIV reservoir, or even help expand it, and that HDACi could put the brain HIV reservoir beyond control. Preliminary results of small pilot studies testing these new strategies are urgently needed.
